Beginning in 2024, ADMT Solutions Home Health partnered with other agencies to start a Homebound Vaccination pilot for Meals on Wheels San Antonio.
The successful pilot program has now allowed us to expand beyond Meals on Wheels clients. If you or your loved one has difficulty leaving the home for vaccines. Contact us for Assistance

- COVID
- RSV
- Shingles
- Pneumonia
- Tdap
Homebound Vaccination Process
1)Homebound individual can submitt the HIPPA compliant secure “Homebound Vaccine Request Form” or Call (210) 729-1252
2)An ADMT representative will call to verify the patient’s information, vaccine needs, health insurance, and schedule a date for the vaccine.
3)An ADMT representative will request the vaccines needed from a partner pharmacy.
4)Vaccines will either be delivered to ADMT or picked up by ADMT.
5)A Registered Nurse (RN) or Licensed Vocational Nurse (LVN) will go to the patient’s home to give the vaccine.
6)The RN or LVN will leave behind additional information and answer other health related questions.
